I'm afraid, as my father used to say "Perfect is the enemy of good"  :)
What you are asking for is a hard problem.  

That said, we live in an age of miracles.  I can think of using a small on device AI to clean up the dictation results.  I made a note of this.  It is an interesting problem, but I don't have time to delve into this in the short term.
