FYI, I got the following  feedback from a user trying to use an external braille keyboard:

Turns out, the external keyboard option is read by VoiceOver only if I tap the middle of the screen then swipe back and forth.

Before figuring out what was happening, I was opening Settings > Input Mode. I then immediately began swiping back and forth wondering why the option for the external keyboard was not presented as explained in the manual.

Within the Mbraille app I'm thinking there must be some sort of weird loss of focus by VoiceOver "hiding" what is obviously on the screen.

I would also like to add that when I immediately open the Mbraille Settings, the other options in this main menu behave the same way (endless swiping from top of screen to bottom then back to the top of the screen).

I need to investigate, but I'm wondering if you ran into this, or something in the way you use VoiceOver navigation circumvents this issue?
