I also have audio ducking turned off. I somewhat agree that it sounds like a voiceover bug would, but I have never experienced it in the legacy MBraille or other situations.

I have seen simular bugs if audio ducking is turned on, so try disable this in VoiceOver. Perhaps that changes something? Sorry, I did not think of this, since I keep audio ducking off here.

I have here a screen recording of the bug where MBraille in some cases does not read a word that is entered. (this is the one I thought was related to keyboard clicks, not the one relating to multi-word contractions.)
Keyboard clicks are turned off, so that must not be the issue.
The words "now" and "see" are entered, but not read out when I swipe right in the video. Upon hearing nothing on a swipe right I do a 1 finger swipe up to demonstrate that the words have been entered properly but not spoken.

I feel like the app is running just a bit more laggy for me than typical right now. That's hard to quantify though. Words are being skipped quite frequently for me this morning, hence my making the video.
I just now (post video) quit the app from app switcher and relaunched. The bug is still happening quite a bit. In this case, it took me typing about three sentences for it to come back. But then it happened twice in a sentence.
